This tutorial was written by me on 15th May 2011
It assumes you have working knowledge of PSP and Animation Shop and it's tools, was written in Version 9, feel free to merge layers as when needed, unless otherwise stated
You will need:
Tube of choice, I am using the work of Paula, at BritsyNin.. you can find the tube here
Scrap Kit 'NurseTG' which you can download from Randysin

Open a new transparent image 600 x 600 and flood fill white
open frame3 and paste as a new layer / resize 50%
select inside the frame using your magic wand / selections - modify - expand 2
open a paper of choice and paste as a new layer / invert and hit delete / drag the paper below the frame
again click inside the frame / expand 2 and open your tube and paste as a new layer / invert and hit delete / change the blend mode to overlay
merge visible these layers and resize 70%
open badge1 and paste as a new layer / resize 50%
open the chart and paste as a new layer / resize 60%
drag the chart to the bottom and position the badge over it, use your eraser tool to erase some of the string so it looks like it's hanging off of the chart
use your selection tool set to rectangle and draw around the photo id box on the badge
open your tube and paste as a new layer and position inside this box, you will need to resize, when happy / invert and hit delete and drag the tube under the badge
repeat the above steps but this time flood fill the box white
move the circle frame to the top left of the tag and drag below the chart and badge / duplicate / mirror and position bottom right
open blood splat2 and resize 75% twice and drag to the bottom
open your main tube and paste as a new layer and position
open staple2 paste as a new layer / resize 20% then resize 50% and position
open wa12 / resize 70% and position to the bottom of your tag
add your copyrights and any text you would like / resize if needed and save
